Speech-Language Pathologist for the Outpatient Therapy Program:
The Outpatient Therapy Program is currently seeking a Texas-licensed Speech-Language Pathologist (CCC or CFY) or Assistant. Candidate should have experience in working with both pediatric and adult aged clients. HCS/TX Home Living Case Manager
Responsible for overall duties of HCS and Texas Home Living Programs waiver programs. Responsible for client supervision in Opportunity Center’s HCS and Texas Home Living Programs.
JOB DUTIES:
JOB REQUIREMENTS: Associate's Degree in Social Work or related field; two years experience in a related setting. Work requires skills to communicate effectively verbally, visually, and in writing; a well-organized, disciplined person who is a self-starter. Ability to integrate social work concepts and functions into the rehabilitation setting; make and implement professional judgments; and establish and maintain positive relationships with clients and families while maintaining total confidentiality. Computer skills are essential. It requires CPR/ First Aid certification and a valid driver's license.

JOB DUTIES:
- Responsible for meeting with clients and families as needed to complete HCS intake process.
- Responsible for planning client activities every week, managing cases, and transporting clients.
- Prepares documentation as required and performs other duties as needed to maintain client files.
- Ensuring LAR’s, parents, families and clients are aware of HCS policy, procedures and duties in regard to contract responsibilities.
- Adhere to all Texas Health and Human Services guidelines regarding HCS policies as well as Bay Area Rehabilitation policies and procedures.
- Demonstrate physical, emotional, and intellectual competencies necessary for self-control and good judgment when performing assigned responsibilities.
- Assess the client's cognitive abilities and physical and emotional needs to determine appropriate interventions.
- Recognize and respect the uniqueness and potential of all participants, their families, and their culture.
- Develop and implement care plans for clients
- Keep records or prepare reports for HHSC or Management concerning clients
- Visit individuals in homes and group settings and provide information on agency services, requirements, or procedures.
- Submit reports and review reports or problems with superiors.
- Advocate for clients to resolve crises.
- Utilize data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure services.
- Confer with clients to discuss plans or progress
- Collaborate with other professionals to assess client needs or plan
- Maintain inventory of supplies.
- Assist with participant’s care & activities as appropriate. To provide training and direction to clients in one-on-one or small group settings. Completes daily/weekly activity logs and ensures that all participants’ files are current. Ensure that appropriate record keeping is maintained for each client, indicating progress or lack of unusual incidents, attendance, punctuality, and other data required.
- Assist in the Development of outcomes and strategies that are unique to the clients and families.
- Provide service coordination as needed and community integration services for enrolled families.
- Maintain a record system to continuously document service coordination activities and direct services provided to the client and family.
- Follow policies/procedures to ensure that the program complies.
- Transports clients to/from Opportunity Center and home safely and efficiently, obeys traffic laws, and monitors vehicle maintenance.
- Ensure privacy, confidentiality, and preservation of clients' dignity when communicating information.
- All other duties as assigned.

HCS Coordinator for the Opportunity Center Program
The Opportunity Center Program is currently seeking a HCS Coordinator/Manager. This person will be responsible for client supervision in the Opportunity Center's HCS Program.
